If you are interested in becoming a dealer of Phoenix USA Inc.’s products, the first step in the process is to complete our credit application. Simply download our form, complete and have the authorized person(s) to sign. Then email to firstname.lastname@example.org or fax to 931-528-2229 for immediate processing.
The following signatures must be provided in order to be considered for Net 30 Day terms:
The signature under the "bank information" section located in the middle of our credit application form. This signature must match the bank’s signature for authorization, or your bank will not respond to our request for credit information.
The "terms" signature located at the bottom of our form. This signature indicates that you agree to our payment terms of Net 30 Day, and that you have provided the most correct and accurate information regarding your company’s credit history.
Please provide an email address for invoices. We no longer send invoices by mail.
